Per DSHS, COVID-19 Vaccine Program providers can now order the COVID-19 vaccination record card from the Texas Department of State Health Services (DSHS). As a reminder, DSHS and the Centers for Disease Control and Prevention (CDC) require all COVID-19 Vaccine Program providers to complete and provide a COVID-19 vaccination record card to every patient/parent/legal representative.
To order the COVID-19 vaccination record cards, submit the following information in the format listed below to the COVID-19 Vaccine Provider Help Desk at COVID19VacEnroll@dshs.texas.gov. Please note, providers may not exceed 50 cards per request and quantities ordered will be tracked by DSHS.

Note: Some organizations may not accept the CDC COVID-19 vaccination record card as an official documentation of receiving the COVID-19 vaccine.
To access patient COVID-19 vaccination record in the Texas Immunization Registry, Immtrac2, Client Immunization Histories may be printed and released to clients by any ImmTrac2 user with the Full Access Provider role. Healthcare providers may use their own release forms and follow their own policies for releasing records to their patients. The Texas Immunization Registry does not need to be notified of these history releases. The Immunization Registry (ImmTrac2) Authorization to Release Official Immunization History, available on the ImmTrac2 forms website, is only needed for clients to request their Immunization History directly from the Texas Immunization Registry, ImmTrac2.
