Case Management for Children and Pregnant Women (CPW) is a case management program that provides health related case management services to Medicaid eligible children with a health condition or health risk and high risk pregnant women. CPW services include assessing the needs of eligible patients, formulating a service plan, making referrals, problem solving, advocacy and follow up regarding patient and family needs. CPW case managers assist patients in accessing needed health related services and benefits. Case managers refer to a variety of resources and coordinate services when needed with community agencies and schools.
CPW providers are licensed social workers or RNs working as individuals or employed by schools, health departments, counseling agencies, health clinics and other types of agencies. Providers are approved through the Texas Department of State Health Services (DSHS) and enrolled with The Texas Medicaid & Healthcare Partnership (TMHP) as Medicaid providers. Case managers have experience assessing the psychosocial needs of children and/or pregnant women, and making community referrals to address their service needs.

Providers are reimbursed $124.15 for the assessment/service plan development and $62.08 for face-to-face follow-up contacts and $23.28 for telephone follow-up contacts.
