The Statewide Association for Regional Medical Home Advancement (STARMHAC) is an exciting new effort being spearheaded by the Texas Children’s Health Plan. The intention of this 3-year HRSA funded project is to build a statewide infrastructure for maternal and child health in the six critical systems outcomes of:
- Family/professional partnerships at all levels of decision making
- Access to coordinated ongoing comprehensive care within a medical home
- Access to adequate private and/or public insurance and financing to pay for needed services
- Early and continuous screening for special health needs
- Organization of community services for easy use
- Youth transition to adult health care, work, and independence
In addition to creating a stakeholder group, STARMHAC will provide training for community partners and offer technical assistance to coalitions interested in developing improvement projects.
